Print and schedule a physical exam with your home medical provider. NOTE: For fall enrollment, if you have had a physical within one year of 6月30日, of the year you are entering SU, or within one year of 12月. 30 for spring enrollment, 您可以要求您的医疗服务提供者完成体检和免疫记录.

Self-Reporting Form:
